NEW CLIENTS, PLEASE INDICATE THE FOLLOWING:
A. PRIVATE INSURANCE: PLEASE PROVIDE THE GROUP NUMBER, POLICY ID NUMBER, AND PRIMARY POLICY HOLDER (name, address & date for birth).
1. ***YOU MUST complete ALL paperwork sent to you via the portal once appt is confirmed.
2. A credit card will need to be put on file for private insurance for co-pays and deductibles or for direct pay.
3. Our staff MUST be able to call and confirm an appt and verify insurance information to secure your selected appt time. Make sure to give us the best number to reach you. (The call may show up as "Private" as our administrative assistants are working from home.
