-If you don’t have a portal account yet or have forgotten your password, please contact your clinician or reach out to our office via phone (810-444-3531) or email crescentcounselingllp@gmail.com
-All portal paperwork must be submitted at least 24 hours prior to your scheduled appointment. If paperwork is incomplete, your appointment may need to be canceled and rescheduled.
-Please note that no-call or late cancellations within 24 hours of your appointment may be subject to a $75 fee.
