Be mindful that requesting an appointment does not guarantee your appointment day or time. Once the appointment is requested, continue without signing in. After you enter and submit your contact information, we will acknowledge the request and you will receive an invitation to the client portal. Once you set up an account with the client portal, Rx ReLeaf will send you a document “client health questionnaire.” Please fill this document out in its entirety as it will be reviewed by our nurse practitioner. Due to the nature of Telehealth, some conditions may be best suited for in-person at Rx ReLeaf’s discretion. If your appointment is approved, you will receive notification by email as well as additional documents to be filled out at least 24 hours prior to your visit.
