This online form is a convenient way to schedule appointments; however, please contact us by phone for additional times. If your appointment has been confirmed, please cancel or reschedule appointment at least 24 hours before the time of the appointment. This gives us the opportunity to fill the available appointment time.
Two (2) or more missed/ cancellations or reschedule may be subjected to the limitations of same day appointment which means you may not be permitted to schedule appointment in the advance. When you schedule an appointment, you are reserving this time and we will not offer this time to another client.
