We no longer allow clients to process payments on the client portal. We require all clients to keep a card on file. To update your card, please complete the "Payment Authorization" form, which is always available on the portal. After you complete this form, you can contact your provider to process your payment.
You can complete a release of information at any time on the portal. The "Release of Information" form is always available on your client portal.
**Please call the office to request a reschedule or cancel an appointment. Please note that No-Contact Absences or Late Cancellations (less than 24 hours notice) for initial appointments with new clients will not be rescheduled. Clients will be charged for No-Contact Absences or Late Cancellations (less than 24 hours notice). Please see the agreement signed at intake or check with your NP for current rates.
