Insurance Benefits: We verify your benefits (in and out of network) as a courtesy, the benefits quoted are not a guarantee, and we will not be certain of your benefits until we receive your first explanation of benefits from your insurance provider. You will be contacted via phone or email regarding this quote so you can make an informed decision about how to pay for your services. We do not verify HMO benefits due to the difficulty of obtaining the verification, please talk to us about how to submit claims on your own, and we will make sure you have all the information you need to do so in a smooth process. Out of network insurance policy holders will be required to pay the full fee at the time of service.
Addresses: The St. Charles office is located at 451 Dunham Road, Suite 400, St. Charles, IL 60174. It is on the northwest corner of Route 64 and Dunham Road. The office is across the street from a Taco Bell for reference. Finding Suite 400: When you walk through the main entrance of the building, walk straight down the hallway and take a right, suite 400 will be on the left side of the hallway and the bathrooms will be on the right side.
The Sycamore office is located at 806 DeKalb Ave, Suite 3, Sycamore, IL 60178. It is a yellow building and our office is on the left-hand side.
Parking: There is plenty of parking surrounding both buildings, it should be easy to find a parking space.
Waiting Room: Please make yourself comfortable and relax in the waiting room until your therapist greets you.
Portal: The first time you login to your client portal, your password is your date of birth (xx/xx/xxxx).
