**NEW CLIENTS** YOUR APPOINTMENT REQUEST WILL NOT BE APPROVED UNTIL ALL OF YOUR INTAKE DOCUMENTS ARE COMPLETED AND WE HAVE RECEIVED YOUR PHOTO ID AND INSURANCE CARD. THERE WILL BE NO EXCEPTIONS FOR THIS. THE DOCUMENTS MUST BE COMPLETED AT LEAST 48 HOURS IN ADVANCE, SO SCHEDULE ACCORDINGLY TO GIVE YOURSELF ENOUGH TIME TO HAVE THEM ALL COMPLETED.
